I dreamt I was working on a painting the other night - quite disappointed to find no sign of it when I woke up.... it's a sign to me that I need to get the brushes out and get on with something (I do have a portrait in progress): I don't think I get blocks so much as periods of a) depression, which I just have to live with until they go away, and b) lack of time ... as I tire more easily than I did when I was a lissom young thing, working on something else leaves me with little energy to get back to the easel.   But this is where the sketch-books come into play - I can usually find something to paint; and failing that, I do another version of something I've already painted, perhaps in a different medium.  A bit of Mozart helps, too - why Mozart?  Don't know - it just trips something in the brain.  Probably we all have our little tricks, and they're well worth cultivating.  
Jim, I hope your arthritis settles down, I think when you are in pain, it can dampen down your creative abilities. Robert, you have reminded me, I need to get some portrait practice in. I've been looking for lessons to show me the basics of portrait painting in oils but I don't seem to have come across them in my area so far.
